You don't need PHP. You basically need any server side language and a REST API to communicate with it. PHP happens to be a popular choice (but I prefer Node.JS). The problem with directly connecting an Android device to a server is that you'll have to hardcode the database credentials into the app, and doing that is bad because even an amateur reverse engineer can easy extract the DB credentials and hack your database.
So the right way to do it is build a REST API or a website and make calls to that website/API using GET/POST requests
